Outreach Case Manager

B SUMMARY

The Front Door Diversion Project offers individuals admitted to a participating hospital’s psychiatric unit who are being considered for possible admission to a Long-Term Care facility, a community-based option to continue their recovery process after discharge from the hospital.

 Primary responsibilities is to provide intake assessments to referrals received from PAS-R. Additionally,

outreach and market to alternative site for referrals to the Front Door Diversion Program

Outreach to Continued Engagement of Consumers Leaving EDs or Inpatient Units without Agreeing to Alternative

Care Options: To increase the likelihood of avoiding institutional level of care, the Provider shall have the capacity to

conduct telephone or face-to-face outreach:

To consumers who were provided recommendations for alternative care options but left the ED or inpatient

unit without agreeing to active services;

o Provide continuing care management for consumers accepting alternative care options until a hard hand-off

is accomplished with the next level care provider.

 

 

JOB DUTIES

 

1.Provide community outreach and engagement via marketing and scheduled meetings with referring entities

2.Submit weekly reports on referrals and outcomes

3.Participate in staff meetings and supervision as scheduled.

4.Attend staff development training and seminars as approved by supervisor.

5.Establish relationships with key individuals within the community, especially as a resource for program participants.

6.Conduct and/or participate in speaking engagements to promote the program, as directed.

7.Secure participant agreements for referral sources

8.Provide community based case management to clients refusing treatment at the time of discharge from an In-patient

setting

9.Other related duties as assigned by manager

 

 

QUALIFICATIONS

 

o Bachelor’s Degree in social services with at least 3 years of related experience

o Knowledge of community/hospital associations/affiliations.

o Excellent communication skills (oral and written communications).

o Well organized, detailed oriented and able to function in a fast-paced work environment.

o Knowledge of computers and applicable software.

o Valid Driver’s license

WORKING CONDITIONS AND PHYSICAL EFFORT:

o Work is mobile

oModerate physical activity. Will require physical effort including frequent walking and occasional lifting up to

25-50 pounds as well as some extended periods of standing

oVery limited exposure to physical risk
