Provide direct services to individuals as specified in their treatment plan.
JOB DUTIES
1.Must be available, as rotation determines, on 24 hour basis, to intervene to crisis needs. Must carry
pager 24 hours daily
2.Must work closely with State Operated Mental Health Facility to plan for services needed for high
rate users referred because they are to be discharged from inpatient care
3.Must work closely with community support treatment team triage
4.Must develop plans to refer, link, and place individuals with community based treatment and other
needed resources
5.Must refer, link, and place individuals with recreation, transportation, financial and medical
entitlement, vocational rehabilitation services, as appropriate and as needed
6.Must coordinate inter- and intra-agency services
7.Must conduct outreach and case findings
8.Must determine eligibility and conduct comprehensive assessment of service needs
9.Must develop the comprehensive master Individual Treatment Plan, with the input of the client
customer and/or family unit
10.Must review Individual Treatment Plan of client customers regularly to assess effectiveness and
need for specific plan elements, again with input of the client customer and/or family unit
11.Must refer, link, and place individual client customers with needed medical health services within the
community
12.Must monitor and evaluate all services provided to individual client customers
13.Must locate appropriate housing facilities as needed
14.Must provide supportive counseling to individual client customers and/or family unit as needed
15.Must provide problem solving assistance to individual client customers and/or family unit as needed
16.Must provide public education on services available and needs of the mentally ill as opportunities
arise and/or as assigned by the Director of Mental Health Case Management
17.Must document all services provided in progress notes and must perform DIS billings of services on
timely basis
18.Performs any other duties as assigned by the Director of Mental Health Case Management
QUALIFICATIONS
Bachelor’s degree in related human services field
Valid Driver’s License and vehicle required to travel
Knowledge of social service and medical needs of psychiatrically disabled persons
Knowledge of available resources to assist them
Knowledge of appropriate record keeping procedures
Sensitivity to multi-cultural differences
Ability to relate well to people of all socioeconomic groups
Knowledge of languages other than English helpful, particularly Spanish
